 I've used a piece of fuel line for the rubber tube. Works just fine.

The rubber tube fits over the threaded rod part and snugged up to
the
knob - it's for cushioning and tension. On the bike frame is a flange
with a tapped hole for the threaded rod (mine has a tapered end for ease
of finding the threaded hole).

Align tab hole over threaded flange hole and screw in threaded rod
until you get some resistance. The rubber tube will put enough tension
on the whole assemble so that it won't come undone as you ride along.

No idea what the nut is for unless it is for the backside of a
non-threaded flange hole.

set of Joe Kenney's repros, which are a long aluminum knob with a 6mm
threaded rod on the end, and a nut. The parts book shows a little
(rubber?) part in between that is NLA.
